Preparing For Your First Appointment
To help make your first appointment with our team as easy and convenient as possible, we allow patients to fill out their new patient forms online before visiting our practice. If you would prefer to fill out your new patient paperwork in our lobby, please plan to arrive an additional 10-15 minutes earlier than you normally would.
Please remember to also bring a valid form of I.D. and your insurance card with you if you have dental insurance. Should you need to reschedule or cancel your appointment for any reason, we ask that you provide us with at least 24 hours’ notice if possible.
